New Patient Forms

New Patient? Complete a New Patient Intake (short) OR a New Patient Packet (Full)

If you are interested in making a new patient appointment,  please complete a Patient Intake Form. (electronic form, no need to print it out, it will take 2 minutes to fill out) Once you submit this electronic form, allow 24 business hours for our staff to call you and schedule your initial appointment.

Once you have made a New Patient Appointment, please complete a New Patient Packet. You can always SKIP completing a Patient Intake Form (above) and fill out the longer version: New Patient Packet. (electronic form, no need to print it out, it will take 15-20 minutes to fill out) Either form will prompt our staff to call you to schedule a new patient appointment.

Established Patient Form

Is there a family member, friend or therapist that you would like to allow communication regarding your visits at MedPsych?

If we need to release records to another provider, company or yourself.

If a member of our staff has asked you  for a copy of the front and back of your insurance card.

If a member of our staff has asked you  for a copy your ID or Driver Licence.

This form is to provide consent from a patient so Medpsych/provider can appeal a drug denial on your behalf.

This form is for established patients who have agreed with their providers to conduct TelePsych appointments.

General Information


  • Practice hours: Mon – Thu 7:20 am- 4:00 pm , Friday 8:00 am – 12:00 pm

  • The office will be closed to observe major holidays.

  • In case of inclement weather, closings will be announced via email/text to patients who have appointments affected by closing.


  New Patients:

  • If you are new to our practice complete a “Patient Intake Form” (above), once that is submitted allow 24 business hours for our staff to call you and schedule.

  • Initial evaluation appointments may last from 40 to 60 minutes, depending on provider preference.

  • If the patient is a minor (under 18), both the child and a parent/guardian must be present for the initial evaluation appointment.

  • Please bring any prior records or reports that you think may be helpful to your clinician to your initial appointment, especially any psychological evaluations.

  • Please arrive  20 minutes prior to your scheduled initial appointment and check in at the front desk.

  • No show or less than 24 hr cancellation of your new patient appointment is subject to a $300 fee. (not covered by insurance)


  Established Patients:

  • Please call our office 919 582 7272, to schedule follow up appointment

  • Please arrive at least 5 minutes prior to your scheduled appointment and check in at the front desk.  If you need to update any insurance or demographic (address, phone number, etc.) information, please arrive at least 10 minutes prior to your scheduled appointment

  • Follow up appointments are scheduled 20 minutes or 40 minutes depending on how much time the provider needs with each patient.

  • Please call at least 24 hours prior if you need to cancel an appointment.  Missed appointments and late cancellations are subject to charges ($75 for 20 min follow up and $150 for 40 min follow up).

  • Please note that if you arrive late for your appointment, you may be asked to reschedule.



  • Patients are required to be seen every 3-6 months in order to obtain refills.

  • Non Controlled substances: call your pharmacy 3-5 days prior to running out of medication and they will send us a refill request.

  • Controlled Substances: call our office and leave a voicemail on your providers line, you may also send a private message through the patient portal.

  • On call service should not be used to request medication refills.
